Your Trusted Partner in Care Management, Advocacy, and Life Transitions
Navigating aging, illness, disability, or major life transitions can feel overwhelming—especially when medical, safety, financial, legal, and family concerns surface all at once. Knowing what to do next or who to trust isn’t always clear.
Diversified Life Care Partners provides professional care management, advocacy, and fiduciary support to help individuals and families move forward with clarity, confidence, and continuity—rather than crisis.
This introductory blog begins a series designed to help families understand who we are, what we do, and when partnering with a professional care manager can make a meaningful difference.
Our Mission: A Holistic Approach to Care and Quality of Life
Our mission is simple and powerful:
to enhance quality of life and support independence at every stage of life.
Founded in 2011 by Ginger Jones, RN, Diversified Life Care Partners was built on more than 30 years of experience navigating healthcare systems, aging services, and complex family dynamics. Originally known as Diversified Nurse Consultants, the organization rebranded in 2025 to reflect the expanded scope and holistic nature of its services.
Over time, a consistent pattern emerged:
care was fragmented, communication was poor, and families felt overwhelmed—often falling through the cracks while trying to manage medical decisions, safety concerns, finances, and long-term planning.
Diversified Life Care Partners was created to fill those gaps.
Today, we deliver client-centered, coordinated support that integrates healthcare, quality of life, legal, financial, and fiduciary needs into one thoughtful, personalized plan.
Our specialty is you.
What Is Care Management?
Care management means having a trusted professional advocate who understands the full picture and coordinates all moving parts.
At Diversified Life Care Partners, care management includes:
-
Comprehensive assessments of medical, functional, emotional, and environmental needs
-
Coordination with physicians, hospitals, home care providers, facilities, and family members
-
Advocacy during medical appointments, hospitalizations, and care transitions
-
Ongoing monitoring and proactive adjustments as needs change
-
Guidance that helps families plan ahead—not react in crisis
-
A consistent, reliable point of contact during complex or emotional situations
Our goal is always the same:
protect quality of life, support independence, and reduce stress for families.
Professional Standards: Aging Life Care Professionals™
Diversified Life Care Partners are members of the Aging Life Care Association® (ALCA) and includes Aging Life Care Professionals™, a nationally recognized credential held by highly trained specialists in aging and complex care.
These professionals come from backgrounds such as nursing, social work, gerontology, counseling, and related fields and must meet strict education, experience, and ethical standards.
All Aging Life Care Professionals™ follow a rigorous Code of Ethics and Standards of Practice, ensuring:
-
Ethical, unbiased advocacy
-
Evidence-based care planning
-
Deep knowledge of local resources
-
Transparency and accountability
-
Consistent, high-quality support
Not all care managers meet these standards—but all Aging Life Care Professionals do.
Fiduciary Services: Advocacy Beyond Care
For individuals aging alone or without an appropriate family decision-maker, support often extends beyond health and daily living.
When appropriate, Diversified Life Care Partners offers professional fiduciary services, including:
-
Power of Attorney
-
Trustee services
-
Executor services
-
Daily money management support
Fiduciary services are provided with transparency, integrity, and strict adherence to the client’s wishes—offering protection, oversight, and peace of mind when trust matters most.
Thoughtful Engagement: Supporting Emotional Well-Being
Aging well is about more than safety and logistics—it’s also about connection, purpose, and dignity.
Our Thoughtful Engagement program provides one-on-one support focused on meaningful activity, companionship, and social connection. This service may stand alone or complement care management and is often an ideal way to begin support before more intensive needs arise.
Who We Serve
Diversified Life Care Partners supports older adults and younger individuals facing disability or serious illness, including those navigating:
-
Aging-related transitions

Dementia and cognitive decline

When Should You Reach Out?
Care management is most effective before a crisis occurs.
You may benefit from partnering with Diversified Life Care Partners if:
-
Care feels disorganized or overwhelming
-
Safety at home is a concern
-
Medical or cognitive needs are increasing
-
Family members disagree on next steps
-
You’re unsure what resources are available
-
You’re feeling burned out or unsure where to turn
-
A loved one has limited or no nearby support
Our role is to act as both coach and advocate, helping families lead with confidence while we provide expertise, coordination, and continuity.
How We Are Different
Diversified Life Care Partners is not a home care agency and is not tied to a single solution.
We take a holistic, unbiased approach—coordinating with trusted community partners when hands-on care is needed while remaining focused on advocacy, oversight, and long-term quality of life.
Clients often describe our work as white-glove care management: high-touch, personalized, and deeply human.
Looking Ahead
This blog is the first in a series designed to educate, empower, and support individuals and families navigating life’s transitions.
If you’re unsure whether you need help, that’s often the right time to start a conversation.
You don’t have to navigate aging—or life’s complexities—alone.
